Job Details:
Function independently to provide high quality patient care in a collaborative and patient-centric manner
Provide care in various settings including hospital, clinic, telephone, video, and secure email when appropriate
Gather patient history, perform physical exams, review prior records and ancillary data
Develop and monitor appropriate treatment plans
Communicate effectively with patients, families, and care team
Provide efficient and compassionate care utilizing a multi-disciplinary approach
Provide relevant education and counseling to support care plans, treatment, and discharge
Collaborate with healthcare team members, leadership, and medical staff
Assist collaborating physicians with hospital patient management
Comply with regulatory standards and practice metrics
Document in accordance with regulatory and care site guidelines
May be required to float to other departments based on business needs
May be required to be placed on-call during regularly scheduled shifts
